Terms and Conditions
for Membership at Excellency Physiotherapy Center LLC
By subscribing to any of the membership tiers offered by Excellency Physiotherapy Center LLC ("theClinic"), you ("the Member") agree to abide by these Terms and Conditions. Please read them carefully.
Payment
Payment Schedule
• Payments for all membership tiers are due upfront each month and are non-refundable.
Payment Methods
• Payments can be made through a secure payment link sent to the Member's registered email address.
Annual Payment
• Members who opt for annual payment will pay upfront for the entire year and receive a 10% discount on the total cost. Annual payments are non-refundable.
Payment Reminder
• The Clinic will send a payment reminder to the Member one day before the expiration of each monthly membership period. Failure to make the payment may result in the suspension or termination of membership services.
Cancellation and Termination
Minimum Commitment
• All membership tiers have a minimum commitment period as specified in the respective membership agreements. Termination of the membership is not permitted before the completion of this minimum commitment period.
Notification Requirement
• Members wishing to terminate their membership must inform the Clinic via email or WhatsApp message at least 1 day before the end of the current billing month. Failure to do so will result in the automatic renewal of the membership for another month.
Exceptions
• Membership discounts do not apply to operations or procedures conducted in the Clinic, such as injections made by an orthopedic surgeon, PRP, Botox, hyaluronic acid treatments, or orthopedic surgeries.
Cancellation Penalties
• If the membership is terminated before the completion of the minimum commitment period, a cancellation fee will be charged. The specifics of this fee will be outlined in the membership agreement.
Timing of Cancellation
• Members must provide notice of cancellation any day before the renewal date of their subscription. Notices received on the renewal date or after will not stop the automatic renewal for the next billing period.
Automatic Renewal
• Subscriptions will be automatically renewed each month unless canceled in accordance with the aforementioned notification requirement.
Confirmation
• Upon receipt of a cancellation request, the Clinic will send a confirmation message to the Member, indicating that the request has been received and processed.
